The outing is a four person scramble with an entry fee of $100 per player. The entry fee includes: a sack lunch, drinks on the course, social hour, evening dinner, putting contest, mulligans, $200 drawing, red tee shot, and contests at each hole. Please print and complete the attached form by listing the name, handicap (if any), company affiliation, and city for each player.

The golf committee is also seeking sponsors for the event. Those interested can help us by sponsoring three different ways: hole sponsorships at $100 each, hole prizes, or goody bag items.
